Registered Nurse / Home Health Hospice

Job Summary:
Home Health Hospice Nurse Missoula, Montana. This role involves providing skilled nursing care, case management, and patient education to individuals in their homes or at an in-patient Hospice Center. The nurse will coordinate services to promote quality clinical and cost outcomes, ensure continuity of care, and collaborate with an interdisciplinary team to meet patient needs.

Job Details:

• Schedule: Dayshift, 8 hours per day, minimum of 40 hours per week, with an on-call rotation.
• Required Qualifications: Current registered nurse (RN) license in the State of Montana, a valid drivers license, appropriate auto insurance, reliable transportation, and a minimum of 0 years of experience in home health, hospice, or acute care settings.
• Preferred Qualifications: BSN degree and experience with clinical skills such as venipuncture, catheterization, wound care, and parenteral therapy are preferred.
• Additional Information: The RN will provide care in residential settings, perform patient assessments, oversee care delivered by aides and certified nursing assistants, and participate in team scheduling and quality improvement activities.
