Registered Nurse / Home Health Hospice

Job Summary:
Home Health Hospice Nurse in Elizabeth, West Virginia. This role involves providing terminally ill patients care in their homes or nursing facilities, primarily covering multiple counties within the region. The position requires adherence to facility protocols and effective coordination with team members to deliver compassionate end-of-life care.

Job Details:

• Schedule: Monday through Friday, 8:00 AM to 4:30 PM, with a minimum of 40 hours per week.
• Required Qualifications: BLS certification (American Heart Association); West Virginia license; a minimum of 2 years of hospice care experience.
• Preferred Qualifications: Hospice experience of one year or greater is preferred.
• Additional Information: Must have hospice experience of 1 year or greater. The role involves caring for terminally ill patients with conditions such as cancer, heart disease, lung disease, and dementia. The setting includes both outpatient and inpatient hospice care, with patient visits typically totaling 5-6 per day during an 8-hour shift. Training includes a specific orientation and system use in EPIC electronic health record system.
• Coverage Area: Harrison, Barbour, Upshur, Lewis, Taylor, Doddridge, Wetzel counties.
• Uniform: Royal Blue scrubs.
• Certification: BLS (American Heart Association) required.
• Experience: Minimum of 2 years of post-acute hospice care experience.
