Job Summary:
The organization is seeking a licensed Registered Nurse (RN) specializing in Case Management for a position in Fort Worth. The role involves providing patient-centered case management services on a day shift, focusing on coordinated care for patients in a neuro step-down unit. This is a temporary assignment designed to support operational staffing needs within the facility.
Job Details:
- Schedule: Day shift, 8 hours per day, with a minimum of 40 hours per week
- Required Qualifications: Licensed Registered Nurse (RN) with at least 1 year of acute care case management experience. Candidates must be familiar with Meditech and Careport electronic medical record systems.
- Preferred Qualifications: Experience with case management in a Neuro Step-Down setting, knowledge of Meditech and Careport systems, and effective collaboration skills.
- Additional Information: The unit has 40 beds, staffed with 1 RN and 1 social worker per shift. The role involves managing a patient ratio of approximately 1:20-30, with a focus on self-starting, solution-focused, and collaborative individuals. There are no on-call or floating requirements.
