Licensed Practical Nurse / Home Healthcare

Job Summary
Home Healthcare (HH) This position involves providing skilled nursing care in a home health setting. The role requires delivery of nursing assessments, medication management, wound care, and disease management to patients in their homes, ensuring quality care aligned with clinical standards.

Job Details

• Certifications: BLS (Basic Life Support) required.
• Education Requirements: Valid Tennessee license or Compact license on file at time of submission.
• Years of Experience: Minimum of 2 years of relevant experience.
• Location Requirements: Must have prior travel experience; reliable transportation is necessary.
• Schedule: Dayshift, Monday through Friday, 8:00 am to 4:30 pm. Minimum 40 hours per week.
• Responsibilities include: Nursing assessments, medication management, wound care, disease management, education, and in-home visits using equipment such as wound vac, IV pumps, neph tubes, trachs, PICC line supplies, port access supplies, Foley catheters, feeding tubes, and ostomies.
• Call Requirement: May have call once a weekend per month; weekend shifts are possible.
• Equipment: Wound Vac, IV pumps, neph tubes, trachs, PICC line supplies, port access supplies, Foley catheter, feeding tubes, and ostomies.
• Visit Duration: Typically 30-45 minutes for visits; up to 1 hour+ for admission visits.
• Productivity: Expectation of 6 patient units per day; efficiency is monitored, with territory assignments to limit geographic coverage.
• Additional Information: No stay in the home for infusions.
• Submission: IVS to interview, manager to extend offers.
