Job Summary
Home Healthcare Nurse (California) This position involves providing in-home nursing services within the home health and hospice setting. The role requires the nurse to perform assessments and deliver patient care based on referrals, ensuring quality and compliance with healthcare standards. Job Details

• Certifications, Education Requirements, Degree Requirements, Location Requirements, and Years of Experience: CA RN license, BLS certification, 1 year of experience in the last 3 years, and 0 years minimum experience required.
• Schedule: Days shift, 8 hours per day, 40 hours per week.
• Responsibilities include setting up in-office referrals for home health and hospice care, utilizing Health Connect (Epic) for charting and documentation.
• Coverage area includes northern California for home health or hospice services.
• Float: Float rotation may be required.
• Submission Requirements: Applications must meet licensing and certification requirements, including a valid CA RN license and BLS certification, and demonstrate recent nursing experience in the last three years.
