Job Summary:
The organization is seeking a Registered Nurse (RN) with a specialty in Case Management to support a high-quality healthcare environment. The role involves coordinating the management of care, ensuring the efficient utilization of resources, and maintaining compliance with external review agencies. The RN will assess patient needs, develop care plans, and serve as a patient advocate, promoting continuity of care and cost-effective management across various acute care settings.
Job Details:
- Schedule: Days shift, 8 hours per day, with a minimum commitment of 40 hours per week.
- Required Qualifications: Valid California Registered Nurse (RN) license; BLS certification from the American Heart Association; a minimum of 2 years recent RN Case Management experience in a Long-Term Acute Care (LTAC) or Short-Term Acute Care Hospital (STACH), including experience with discharge planning, care coordination, and interdisciplinary team collaboration.
- Preferred Qualifications: Experience in a similar facility setting, strong understanding of care management processes, and excellent communication skills.
- Additional Information: The role requires comprehensive assessment and care coordination, focusing on patient needs while addressing holistic considerations, discharge planning, and resource utilization. Candidate must use the specified template for submissions, and incomplete or non-compliant submissions will be rejected.
- Schedule: The assignment will run from 10/26/2026 through 01/25/2027.
