Job Summary:
Case Management Specialist in Kansas City, Missouri. This role involves providing discharge planning and care coordination services within a facility equipped with documentation systems such as Meditech, Midas, and WellSky. The position requires collaboration with clinical staff to ensure optimal patient outcomes in a busy hospital setting.

Job Details:

• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week. The schedule runs from 10/19/2026 to 01/11/2027, with rotating weekends.
• Required Qualifications: Current license to practice as a Registered Nurse (RN) or Social Worker (SW), recent acute care discharge planning experience, and ability to adhere to business casual dress code.
• Preferred Qualifications: Experience with documentation systems such as Meditech, Midas, and WellSky is preferred.
• Additional Information: This role may require floating to any unit within the hospital as needed. Candidates should be comfortable working in a fast-paced environment handling discharge planning responsibilities. The position involves providing dedicated case management services to a patient population across a unit with 590 beds and varying patient ratios.
