Job Summary:
Social Worker in a healthcare setting, responsible for coordinating patient care and services for individuals requiring assistance or meeting Care Management criteria. This role involves collecting comprehensive information about patients` medical, functional, and social conditions to develop personalized care plans. The Social Worker collaborates with patients, families, and the healthcare team to determine goals, coordinate discharge planning, and ensure the appropriate utilization of community resources. The position emphasizes improving patient outcomes, advocating for patient rights, and supporting efficient resource use within the organization.

Job Details:

• Schedule: 8:00 AM to 4:30 PM, 40 hours per week
• Required Qualifications: Graduate of an accredited school of social work, Sociology, or related field; LCSW license in Montana; minimum of one year clinical experience in a healthcare setting.
• Preferred Qualifications: Two years of case management experience in a hospital setting; ACM or CCM certification (not required).
• Additional Information: The Social Worker will participate in multidisciplinary teams, facilitate rounds and case conferences, and promote the organization`s mission and values. The role involves assessing patient needs, developing comprehensive care plans, and coordinating services to optimize patient care quality and efficiency.
