Registered Nurse / Case Management Exp

Job Summary:
Case Management Exp is a clinical specialty role located in a healthcare facility. This position involves coordinating patient care and services for individuals requiring assistance or meeting Care Management criteria. The role includes comprehensive assessments of patients` medical, functional, and social conditions, development of individualized care plans, discharge planning, and collaboration with the healthcare team to optimize patient outcomes.

Job Details:

• Qualifications include RN licensure in Montana or via compact, BSN degree, and a minimum of 0 years of experience in a hospital care management setting.
• Schedule: Day shift, 8 hours per day, 40 hours per week.
• Certifications Required: BLS (CPR).
• Additional responsibilities encompass organizing resources, advocating for patient rights, participating in multidisciplinary meetings, and monitoring service utilization and outcomes.
• Preferred qualifications include five or more years of nursing experience and certifications such as ACM or CCM, though these are not mandatory.
• The role requires strong knowledge of medical terminology, proficiency in desktop software including PowerPoint and Excel, and effective data entry skills.
• Must be able to perform essential duties satisfactorily and adapt accommodations if necessary.
