Registered Nurse / Home Healthcare

Job Summary
Home Healthcare Registered Nurse (RN) position is located in a specified territory within the area. The role involves providing home health services to geriatric patients, performing assessments, wound care, medication management, and other skilled nursing duties. The position is suited for an experienced RN with knowledge of OASIS and proficiency in the HCHB (Homecare Homebase) system, requiring a minimum of 1 year of relevant experience. The schedule includes 8-hour shifts, with at least 40 hours per week, starting on 10/05/2026.

Job Details

• Schedule: 8-hour shifts for a minimum of 40 hours per week, beginning on 10/05/2026 and ending on 01/04/2027.
• Required Qualifications: Valid nursing license in hand, minimum of 1 year of relevant home health experience, OASIS experience required for staff completing Start of Care (SOC), proficiency in HCHB (Homecare Homebase), current BLS (Basic Life Support) certification, active driver`s license with personal transportation, and recent experience with adult home health assessments and procedures.
• Preferred Qualifications: Experience with discharge planning, recertification, medication management, and wound care, along with the ability to perform routine visits and start of care and resumption of care assessments.
• Additional Information: The role requires flexibility with territory, timely documentation, and communication. All charting must be completed by 9 a.m. the following morning. The traveler is expected to be adaptable in covering up to a 60-mile radius, with a focus on geriatric patients. On-call rotation and weekend coverage may be required; discussions to be held during interview.
• Certifications: BLS required.
• Qualifications for Re-Applicants: Must be separated from previous employment for a minimum of 6 months, with HR approval required for re-entry.
