Job Details

• Schedule: Days shift, 8 hours per shift, with a minimum of 40 hours per week.
• Required Qualifications: Bachelor`s Degree from an accredited college or university; Nursing (RN) license valid in the state or a compact jurisdiction; minimum of 0 years of experience.
• Preferred Qualifications: Experience in case management or discharge planning is a plus.
• Additional Information: Responsibilities include coordinating patient care and discharge, acting as an educational resource for staff and families, leading administrative support staff, participating in quality improvement initiatives, and collaborating with interdisciplinary teams to assess and plan patient care needs. The role requires the ability to evaluate data, develop plans of care, and ensure compliance with federal and state laws governing healthcare operations. The position involves active participation in department and hospital committees and may require leadership of clinical or administrative support staff.
