Registered Nurse / Case Management Exp

Job Summary
The specialized case management professional will coordinate patients through the acute care hospitalization process at a healthcare organization. This role involves managing all aspects of discharge planning and transition, as well as performing utilization management and quality screenings. The position requires a proactive approach to patient care coordination within an inpatient hospital setting.

Job Details

• Schedule: 8:00 AM to 4:00 PM, with a minimum of 37.5 hours per week. Rotating weekends: every 4th or 5th weekend. Holidays include one summer and one winter holiday.
• Required Qualifications: Bachelor`s Degree in Nursing (BSN) required, RN license required, with at least 2 years of experience in acute inpatient hospital discharge planning. Experience with subacute patients is also required.
• Preferred Qualifications: Certified Case Manager (CCMC) preferred.
• Skills: In-Patient Case Management, Case Management, Managed Care, Acute Care Experience.
• Experience: A minimum of 0 years required, with a strong preference for at least 2 years of experience in acute care setting discharge planning.
• Start Date: 10/19/2026
• End Date: 01/16/2027
