Registered Nurse / Case Management Exp

Job Summary:
Case Management Nurse (CM EXP) in a healthcare setting. This role involves providing coordinated patient care, facilitating communication among the care team, and managing complex discharge planning for a diverse patient population. The position focuses on ensuring smooth transitions of care from pre-admission to post-discharge, particularly for complex, high-utilization, and vulnerable patients.

Job Details:

• Schedule: Nights shift from 8:00 PM to 4:30 AM, Tuesday through Saturday, with a minimum of 36 hours per week.
• Required Qualifications: Bachelor`s Degree in Nursing (BSN) required; Registered Nurse (RN) licensure in the state of practice required; Minimum of 1 year of experience in inpatient case management required; Basic Life Support (BLS) certification from approved vendor required.
• Preferred Qualifications: Experience in a care management role preferred; Case management certification preferred; Staff familiar with strike force for health plans such as Kaiser Permanente or HCA California, requiring minimal training preferred.
• Additional Information: The candidate must demonstrate the ability to handle complex discharge planning, collaborate effectively with multidisciplinary teams, and coordinate patient transfers to tertiary care centers when necessary. Knowledge of regulatory and accreditation compliance data collection is essential.
