Registered Nurse / Case Management Exp

Job Summary:
Case Management Nurse Specialist in Pasadena, TX. This role involves providing case management services within a hospital setting, focusing on discharge planning and patient status verification. The position requires adaptability to various medical-surgical units and a collaborative approach to patient care.

Job Details:

• Schedule: Day shift, 8 hours per day, minimum of 40 hours per week
• Required Qualifications: Valid RN license; 2 years of hospital case management experience, with experience in discharge planning and interqual criteria assessment.
• Preferred Qualifications: Candidates with 2 years hospital case management experience are prioritized.
• Float: Float rotation may be required to any medical-surgical or ICU unit if needed.
• Additional Information: The role covers the entire hospital`s case management and social work department, overseeing patients across multiple units. No weekend or on-call duties are required. Position is for two openings, with a projected start date of 10/19/2026 and end date of 01/18/2027.
