Registered Nurse / Case Management Exp

Job Summary:
The healthcare professional specializes in Case Management and will be based in an acute care setting. This role involves coordinating patient care management, ensuring optimum resource utilization, and supporting discharge planning processes. The position emphasizes collaboration across interdisciplinary teams to improve patient satisfaction and promote quality of care.

Job Details:

• Schedule: Days, 8-hour shifts, minimum of 40 hours per week
• Required Qualifications: California RN license, BLS certification through the American Heart Association, minimum of 2 years recent RN case management experience in a long-term acute care or skilled nursing facility setting, experience with discharge planning across acute care environments, strong interdisciplinary team collaboration skills
• Additional Information: The position requires comprehensive assessment, care coordination, care plan implementation, and evaluation of individual patient needs while incorporating patient preferences. Candidates must follow facility protocols and demonstrate effective resource utilization to serve as a patient advocate. Submissions without the required template will be rejected.
