Job Summary:
Case Management EXP (Specialty) The position is located in an acute care facility. The role involves assessing patients, coordinating care, discharge planning, and utilization review. The ideal candidate will have hospital experience and be licensed to practice in the state. The assignment starts as soon as possible and is scheduled from October 5, 2026, through April 5, 2027, with a standard weekly schedule of 40 hours over 8 hours per shift.
Job Details:
- Schedule: 8-hour shifts, minimum of 40 hours per week
- Required Qualifications: Valid state license (compact license accepted), at least 2 years of hospital experience, and current clinical experience working in acute care case management or discharge planning.
- Preferred Qualifications: Experience with HMOs, IPAs, care coordination skills, and knowledge of complex patient populations including mental health or patients in crisis.
- Float: Float rotation may be required.
- Additional Information: The position requires experience with assessing patients, care coordination, discharge planning, and utilization review. Candidates must be able to organize and participate in family meetings, develop treatment plans, and manage DME, SNF, and ambulance transport arrangements independently. Documentation must be completed using computerized systems, such as Epic. The assignment involves working holiday shifts as scheduled and the ability to meet department needs, including weekend coverage every other weekend. All scheduling and RTO requests need to be communicated during submission.
