Registered Nurse / Case Management Exp

Job Summary:
The Case Management experience (Specialty: Case Management Exp) position is based in an acute care facility on the dayshift with a schedule of 8 hours per day. The role involves coordinating discharge planning, utilizing care management skills, and working within a dynamic healthcare environment to support patient flow and care continuity. This assignment is suitable for qualified healthcare professionals with relevant experience in case management.

Job Details:

• Schedule: Dayshift, 8 hours per day, minimum of 40 hours per week, start date 10/05/2026, end date 01/04/2027.
• Required Qualifications: At least 0 years of experience in case management or related roles, active state license in California (or eligible pending license), and valid hospital experience (behavioral health experience does not apply). Must have experience with assessing patients, discharge planning, care coordination, and utilization review.
• Preferred Qualifications: Experience with HMO, IPA, and working with complex patient populations including those experiencing homelessness or with mental health issues. Knowledge of DME arrangements, SNF, and ambulance transport preferred.
• Additional Information: The role requires familiarity with electronic charting systems such as Epic and the ability to collaborate with interdisciplinary teams including physical therapy and social services. Flexibility to meet department needs, including weekend coverage every other weekend, is expected. Completed clinical or operational modules may be required before or during the assignment.
