Registered Nurse / Case Management Exp

Job Summary:
Case Manager with specialization in Case Management Experience is needed in an acute care setting. This role involves providing ongoing support through comprehensive patient assessments, care planning, discharge coordination, and collaboration with multidisciplinary teams to improve patient outcomes and satisfaction.

Job Details:

• Schedule: Rotating shift, 8 hours per shift, minimum of 40 hours per week
• Required Qualifications: Valid nursing license in the state of North Carolina, Nursing Diploma or Associate Degree in Nursing (ADN/ASN) from an accredited school, at least 2 years of healthcare experience as a Registered Nurse, strong assessment and critical thinking skills.
• Preferred Qualifications: Bachelor of Science in Nursing (BSN) or Master of Science in Nursing (MSN) from an accredited school.
• Additional Information: The role requires experience in hospital care where patients are acutely ill with complex medical needs. Candidates should have proficiency in care planning, utilization review, discharge planning, and experience with electronic health records systems. The position involves active participation in multidisciplinary meetings, patient education, and coordination of post-discharge services.
