Job Summary:
Case Management Specialist in a healthcare organization located in California. This position involves managing discharge planning, care coordination, and ensuring compliance with evidence-based guidelines for a diverse patient population. The role requires collaboration across interdisciplinary teams to facilitate optimal patient outcomes during a temporary assignment.

Job Details:

  • Shift: Day shift, 8 hours per day
  • Minimum Hours Per Week: 40 hours
  • Start Date: 10/05/2026
  • End Date: 01/04/2027
  • Required Qualifications: 2 years of acute care case management or discharge planning experience, valid California license (pending accepted), at least 1 year of travel or temporary healthcare staffing experience, proficiency with Epic electronic charting system, ability to assess and coordinate care for diverse patient diagnoses, and compliance with COVID-19 and flu vaccination policies.
  • Preferred Qualifications: Experience with inpatient admission criteria, behavioral health, utilization management, and working with HMOs, Medicare, Medi-Cal. Knowledge of CMS (Centers for Medicare and Medicaid Services) guidelines and ability to perform independently with DME (Durable Medical Equipment), SNF (Skilled Nursing Facility), and ambulance transports.
  • Additional Information: This role entails assessing patients, organizing care plans, and participating in family meetings to develop treatment strategies. Candidates must be able to work independently and coordinate discharge plans efficiently. This is a long-term temporary assignment with specific scheduling requirements, including working three out of five major holidays and being available for weekend shifts.
  • Submittal Requirements: Candidates must include responses regarding past employment at a healthcare facility, references covering the last 3 years, and confirmation of licensure and experience.
