Job Summary:
Case Management Specialist in the specified location. This role involves providing care coordination within an acute care setting, requiring prior experience with California Dignity/CommonSpirit health systems. The position focuses on patient management and safety, with a core responsibility to ensure smooth patient flow and adherence to clinical standards for a defined period.
Job Details:
- Schedule: Day shift, 8 hours per shift, with every-other-week coverage, minimum of 40 hours per week.
- Required Qualifications: Valid California Registered Nurse (RN) license, Basic Life Support (BLS) certification, LA City Fire & Safety certification, and a minimum of 2 years of relevant experience in case management or care coordination.
- Preferred Qualifications: Prior experience within California Dignity/CommonSpirit health systems; solid background in care coordination and acute care protocols.
- Additional Information: Must have prior experience with California Dignity/Common Spirit facilities or equivalent health systems. The role requires candidates to be flexible with floating to different units as needed.
