Registered Nurse / Home Healthcare

Job Summary:
Home Health Registered Nurse (RN) in a community setting. The role involves assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Responsibilities include performing admission processes and case management tasks in accordance with organizational policies and standards of community health practice. The position requires thorough and timely electronic documentation and effective interdisciplinary communication.

Job Details:

• Shift Time: Days
• Schedule: 8 hours per day, minimum of 40 hours per week
• Required Qualifications: Valid license or certification; minimum of 1 year Certified Home Care experience; 1 year Case Management Experience; experience in hospice, private care, or working as a Case Manager outside of a certified home health agency does not meet the qualification criteria; ability to travel to cases as assigned; weekend rotations approximately once every 6 weeks.

• Additional Information: Start date is contingent upon credentialing clearance; if not cleared by the posted start date, a rescheduled start date will be necessary.
