Registered Nurse / Case Management Exp

Job Summary:
This healthcare professional position in case management is located in Atlanta, Georgia. The role involves reviewing skilled nursing assessments and developing safe discharge plans for patients transitioning from acute inpatient settings, skilled nursing facilities, long-term acute care (LTAC), and inpatient rehabilitation facilities.
