Registered Nurse / Hospice

Job Summary:
The Roanoke-based hospice care organization is seeking a Registered Nurse (RN) Care Manager for a full-time position. This role involves providing compassionate, patient-centered nursing care within patients` residences, including symptom management, crisis intervention, and family education. The RN Care Manager collaborates with an interdisciplinary team to ensure quality end-of-life care and support for patients and their families.

Job Details:

• Location: Roanoke, Virginia
• Schedule: Day shift, 8 hours per shift, minimum 40 hours per week
• Required Qualifications: One or more years of experience in hospice care, valid Virginia drivers license, good driving record, reliable transportation, and a current Virginia nursing license.
• Preferred Qualifications: Experience working in home hospice settings, excellent communication and problem-solving skills, and ability to work well with interdisciplinary teams.
• Additional Information: The role primarily involves working in patients` residences with travel to long-term care facilities, hospitals, and other community locations within the service area. Overnight travel outside the service area may be necessary for continued education. Employees are at direct risk for exposure to infections and bloodborne pathogens.
