Registered Nurse / Home Healthcare

Job Summary:
Home Healthcare Registered Nurse position in Lorain, Ohio. This role involves providing in-home care to geriatric patients, with a schedule of Monday through Friday, 8:00 AM to 5:00 PM. The position requires flexibility with territory and offers the opportunity to work within a team dedicated to delivering quality home health services.

Job Details:

• Location: Lorain, Ohio
• Schedule: Monday - Friday; 8 hours per day, minimum of 40 hours per week
• Required Qualifications:
  • One (1) year of relevant experience in home health care
  • Active license in Ohio
  • Proficiency in Homecare Homebase (HCHB) required
  • Experience with OASIS assessments required for staff completing Start of Care (SOC)
  • BLS certification required
  • Recent experience with geriatric patient populations preferred
• Preferred Qualifications:
  • Community hospital experience
  • Home health or hospice experience preferred
• Additional Information:
  • On-call rotation may be required, to be discussed during interview
  • Timely documentation and communication are mandatory
  • Mileage will be reimbursed at IRS standard rate
  • All travelers must have an active driver`s license and personal transportation
  • RTO (Requested Time Off) must be submitted at the time of application
  • No first-time travelers accepted
  • All candidates must be flexible with territory within up to a 60-mile radius of the designated hub/office
  • Orientation provided on Mondays, with mandatory modules to be completed during orientation
