Job Summary
Case Management Specialist in Largo, Florida. This position involves coordinating patient discharge plans, utilizing electronic medical record systems, and collaborating with interdisciplinary teams. Job Details

• Schedule: Day shift, 8 hours per shift, with a minimum of 40 hours per week.
• Required Qualifications: Certification or experience in case management, experience with Meditech and Midas electronic medical record systems, candidates with at least 1 year of relevant experience.
• Preferred Qualifications: Experience with complex discharge planning.
• Additional Information: The assignment involves working every other weekend and rotating holidays in accordance with facility policy. The unit is part of a psych/rehab department, and the traveler will be scheduled to support this unit as needed, including float requirements between units.
