Job Summary:
The healthcare organization is seeking a Case Manager Nurse in Chapel Hill, North Carolina. This role involves providing comprehensive assessment, care planning, and discharge coordination for acutely ill patients with complex medical needs. The position aims to enhance patient outcomes and promote continuity of care across various clinical settings.

Job Details:

• Schedule: Rotating shift, 12 hours per shift, minimum of 36 hours per week
• Required Qualifications: Two (2) years of healthcare experience as a Registered Nurse; strong assessment and critical thinking skills; licensed to practice as a Registered Nurse in North Carolina
• Preferred Qualifications: N/A
• Additional Information: The candidate must have experience working as a care manager or discharge planner in an acute hospital setting, managing patients with complex medical conditions, and utilizing electronic health records systems. Responsibilities include conducting patient assessments, participating in multidisciplinary meetings, coordinating discharge plans, and educating patients and families on community resources. The role requires adaptability to change, excellent organizational skills, and adherence to departmental and regulatory guidelines.
