Job Summary:
This position is for a Clinical Nurse specializing in Case Management in the North Carolina area. The role involves providing comprehensive assessment, planning, and coordination of patient discharge and transition processes within an acute care hospital setting. The Care Manager plays a vital part in ensuring optimal patient outcomes, promoting seamless care progression, and facilitating effective communication among multidisciplinary teams.

Job Details:

• Schedule: Rotating shifts, 8 hours per shift, minimum of 40 hours per week
• Required Qualifications: Two (2) years of healthcare experience as a Registered Nurse; Strong assessment and critical thinking skills; Licensed to practice as a Registered Nurse in North Carolina
• Preferred Qualifications: BSN or MSN from an accredited school of Nursing
• Additional Information: The incumbent will support complex patient discharge planning and transition management. Responsibilities include attending multidisciplinary meetings, facilitating communication with patients and families, coordinating referrals and post-discharge services, and assisting with medication management. The role requires compliance with departmental policies and regulatory standards, adaptability to change, and attention to detail.
