Job Summary:
Licensed Clinical Social Worker (LCSW) in Los Angeles, California. The position involves providing case management services within a designated healthcare setting. The role requires collaboration with healthcare teams to develop and implement patient care plans, ensuring compliance with facility policies and industry standards.
Job Details:
- Shift Schedule: Nights, 12 hours per shift, with a minimum of 36 hours per week
- Required Qualifications: Minimum of 2 years of clinical experience in case management, valid California RN license, ACLS and BLS certifications (must be attached to the candidate submission), vent/trach experience.
- Additional Information: Candidates must utilize the specified submission template, including details such as years of travel experience, EMR experience, and certifications on file. Submissions without the template will be rejected. Candidate SSN, DOB, RTR, and vendor submittal packet must be attached to the candidate record. Indicate if the candidate has requested time off within the past three months and provide available interview times over the next 72 hours.
