Registered Nurse / Case Management Exp

Job Summary:
Registered Nurse (RN) specializing in Case Management in Palm Harbor, Florida. This role involves coordinating and providing supportive, end-of-life care to terminally ill patients and their families. The RN will utilize assessment, intervention, and teaching skills to enhance patient comfort and quality of life while collaborating closely with the interdisciplinary team and supporting community education initiatives.

Job Details:

• Schedule: Day shifts, 8 hours per day, minimum of 40 hours per week.
• Required Qualifications: Valid Florida license as a Registered Nurse; Bachelors degree or Associate`s degree from a two-year college or technical school; Minimum of 1 year of related experience; Valid Florida State Drivers License; BLS certification from the American Red Cross or American Heart Association; Reliable transportation with proof of automobile insurance.
• Preferred Qualifications: Prefer one year experience in an acute or post-acute care setting; experience caring for terminal patients and individuals with life-threatening illnesses; ability to type 25 words per minute.
• Additional Information: The position involves providing comprehensive patient assessments, developing and updating plans of care aligned with patient and family goals, conducting patient and family education, ensuring safety and infection control protocols, and supervising licensed practical nurses and home health aides. Participation in team meetings, documentation compliance, medication administration, and community outreach are integral parts of the role. The assignment is based within a teams geographical territory in Pinellas County, with no on-call or weekend visits required.
