Job Summary
The facility is seeking a Home Health Registered Nurse (RN) in Lacey, Washington. This role involves providing comprehensive home health care services with a focus on geriatric patients. The nurse will coordinate, deliver, and document patient care, ensuring timely and effective treatment in a home setting.
Job Details
- Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week.
- Required Qualifications: Valid license in hand with at least 1 year of relevant experience in home health care, proficiency with the Homecare Homebase (HCHB) charting system, and OASIS experience for staff completing Start of Care (SOC). Additional skills include adult home health, admission assessments, wound care, medication management, IV start and maintenance, infection control protocols, and pain management.

- Float: Float rotation may be required.
- Additional Information: The position requires flexibility within a territory of up to a 60-mile radius from the hub/office, with timely documentation and communication expected. All charting must be completed by 9 a.m. the following morning. The role involves weekly visits, primarily geriatric cases, and requires an active drivers license and personal transportation. Orientation occurs on Monday. RTO requests should be submitted with the application, and on-call rotation may be required. Former permanent employees must have been separated for at least 6 months before reapplying as travelers. Each traveler will be assigned to their own territory.
