Registered Nurse / Case Management Exp

Job Summary:
The healthcare provider is seeking a qualified Registered Nurse with case management experience for a position in Chapel Hill, North Carolina. This role involves providing comprehensive assessment, care planning, discharge coordination, and collaboration with multidisciplinary teams to support acutely ill patients with complex medical needs. The position requires a detail-oriented professional capable of adapting to dynamic clinical environments to improve patient outcomes and satisfaction.

Job Details:

• Schedule: Rotating shift, 12 hours per shift, minimum of 36 hours per week
• Required Qualifications: Two (2) years of healthcare experience as a Registered Nurse, strong assessment and critical thinking skills, Nursing Diploma or Associate Degree in Nursing (ADN/ASN) from an accredited school, licensed to practice as a Registered Nurse in North Carolina
• Preferred Qualifications: Bachelor of Science in Nursing (BSN) or Master of Science in Nursing (MSN) from an accredited school of Nursing
• Additional Information: Candidates will be responsible for supporting patient care progression, discharge planning, coordination of community resources, and multidisciplinary team collaboration. Prior experience in hospital acute care settings with complex medical cases is preferred.
