Registered Nurse / Case Management Exp

Job Summary:
Registered Nurse (RN) specializing in Case Management (CM EXP) is sought for a facility in Dover, New Jersey. The role involves coordinating and facilitating patient care through effective collaboration with interdisciplinary care team members, ensuring optimal resource utilization, service delivery, and compliance with external review agencies. The position requires engaging in comprehensive assessment, care planning, and discharge planning to promote continuity of care, patient satisfaction, and cost-effectiveness within a long-term acute care or short-term acute care setting.

Job Details:

• Schedule: Dayshift, Monday through Friday, 8:00 AM to 4:30 PM, minimum 40 hours per week
• Required Qualifications:
  • Valid New Jersey RN license or multi-state RN license (must be attached to resume)
  • BLS certification through the American Heart Association (must be attached to submission packet)
  • Minimum of 4 years recent RN Case Management or Licensed Clinical Social Worker experience, with experience in a Long-Term Acute Care (LTAC) or Short-Term Acute Care Hospital (STACH) setting, discharge planning across acute care settings, and strong interdisciplinary team collaboration
• Additional Information:
  • Must adhere to facility-specific protocols and standards
  • Shift is Monday through Friday, 8:00 AM to 4:30 PM
