Registered Nurse / Case Management Exp

Job Summary:
A healthcare organization in Indiana is seeking an experienced inpatient case manager for a day shift assignment. The role involves working within a hospital utilization management team, coordinating patient assessments, driving stay progression, and expediting discharge planning. The position requires collaboration with the care team and proficiency in electronic documentation systems.

Job Details:

• Schedule: 8-hour shifts, Monday to Friday, with participation in weekend rotation. Minimum of 40 hours per week.
• Required Qualifications: Indiana Registered Nurse (RN) license or compact, a minimum of 2 years of inpatient case management experience, proficiency with Epic documentation, and strong knowledge of hospital utilization management, InterQual, and MCG.
• Preferred Qualifications: Certification in Case Management.
• Float: Will be primarily assigned to one unit but may need to float to other units/departments performing inpatient case management work.
• Additional Information: Orientation includes 6 hours of general hospital orientation, 4 hours of Epic training, and 8-16 hours of peer-specific training. This assignment is located in Fort Wayne, Indiana, at a facility with units ranging from 20-36 beds. Clinicians will be expected to participate in a weekend rotation and follow business casual dress code, with optional Eggplant-colored scrubs or white lab coat. A non-billable orientation of 8 hours will be provided, and clinicians may be called off with 24-hour notice during the contract. An Indiana RN license or equivalent is required prior to start.
