Job Summary:
The healthcare organization is seeking a Social Worker for a Post-Acute Care setting in St. Paul, Minnesota. This position involves providing social work services primarily during day shifts, with a 13-week assignment duration. The role requires collaboration with multidisciplinary teams to support patient needs and facilitate discharge planning.

Job Details:

• Position: Social Worker
• Location: St. Paul, Minnesota
• Shift: Days, 8:00 AM to 4:30 PM; no weekends or holidays
• Schedule: 40 hours per week
• Start Date: 08/03/2026
• Certifications: BLS Certification required
• Experience: Minimum of 2 years of experience in social work
• Required Qualifications: Bachelors degree in Social Work from an accredited curriculum, valid Minnesota social work license, at least 2 years of experience, and the ability to submit licensing documentation
• Additional Information:
  • Length of assignment: 13 weeks
  • Weekly guaranteed hours: 40
  • On call and call back are not billable at the facility
  • No charge differential at facilities; local rates apply if housing is used
  • Clinical guarantee: 36 hours per week for twelve-hour shifts, or 40 hours for eight to ten hour shifts
  • Charting system: Point Click Care (PCC)
  • Cancelation policy: 2 weeks` notice required
