Job Summary:
Case Management Specialist in California. This position requires a highly experienced professional with expertise in acute hospital case management and discharge planning. The role involves assessing patient needs, coordinating care, and facilitating smooth discharge processes within an acute care setting.
Job Details:
- Schedule: Days shift, 8 hours per day, minimum of 40 hours per week.
- Required Qualifications: Must have hospital experience, at least 2 years in acute care case management or discharge planning, current California license (pending license accepted but must be licensed upon start date), and at least 2 years of relevant experience.
- Preferred Qualifications: Experience with HMOs, IPAs, and familiarity with assessment, care coordination, discharge planning, utilization review, and working with complex patient populations.
- Patient Ratios: 1:25 on days, 1:45 on weekends.
- Weekend Coverage: Rotational weekend shifts required, approximately every other weekend.
- Float: Travelers are expected to float among facilities within the state; refusal to float may lead to assignment termination.
- Additional Information: The position involves working in an acute care environment with a case load of up to 25 patients. Candidates must demonstrate experience with assessment, care coordination, discharge planning, and familiarity with clinical and operational procedures. Expect to perform tasks independently such as arranging DME, SNF, and ambulance transport, and developing treatment plans with multidisciplinary teams. An average of 16 hours of non-billable module training is required, with completion recorded in Workramp transcripts.
