Job Summary
The facility is seeking a Registered Nurse (RN) Case Manager in Winchester, Virginia. This role involves providing case management services to adult patients, focusing on coordinating and managing patient care within a Level II Trauma Center. The position requires a dedicated professional capable of handling Medicare, Medicaid, and commercial patient populations, ensuring quality and safety in patient outcomes.

Job Details

  • Schedule: 8:00 a.m. to 4:30 p.m. (five days a week), minimum of 40 hours per week.
  • Required Qualifications: Valid Virginia State License or Compact license, Basic Life Support (BLS) certification from the American Heart Association, 2+ years of experience in case management, and experience with EPIC electronic medical records.
  • Preferred Qualifications: Experience specifically in case management involving Medicare, Medicaid, and commercial patients.
  • Float: Float rotation may be required within the Care Management Unit.
  • Additional Information: Position is located at a facility with 20-30 beds and an average patient ratio of 1:20 to 1:30. The unit handles adult patients in a busy trauma center setting, with a focus on safety and quality care protocols. Effective communication and professional attire in scrubs (eggplant purple) or business casual are expected. Candidates may be considered if located within a 50-mile radius of Winchester, Virginia, and must be well-qualified with all required experience.
