Registered Nurse / Home Healthcare

Job Summary:
A Palliative Care Case Manager position is available in the home healthcare specialty within the Martinez area. The role involves providing patient visits in their homes, skilled nursing facilities, or assisted living communities. The working schedule is primarily Monday through Friday, 8 a.m. to 5 p.m., with a combination of patient visits and administrative tasks in the office.

Job Details:

• Location: Onsite in Martinez, with visits to patients in their homes, skilled nursing facilities, or assisted living communities.
• Schedule: Dayshift, 8 hours per day, minimum of 40 hours per week.
• Required Qualifications: Valid healthcare license and certifications, including Basic Life Support (BLS). Recent (within the last 3 years) full-time home health experience totaling at least 1 year.
• Preferred Qualifications: Basic assessment skills and reporting experience; ability to communicate effectively with practitioners or physicians via SBAR.
• Additional Information: The position entails clinic/charting work different from electronic medical record systems used in in-home health. The clinician will perform assessments, report results, and triage patient needs as required. The assignment duration is approximately 13 weeks. Candidates should be prepared to work onsite and complete in-person visits accordingly.
