Registered Nurse / Hospice

Job Summary:
Hospice Nursing in Grand Junction, Colorado. This position involves providing compassionate end-of-life care in patients` private homes, nursing homes, group homes, or assisted living facilities. The hospice nurse acts as both caregiver and educator, guiding patients and families through personal goals of care with a focus on symptom management and holistic support.

Job Details:

• Schedule: Monday through Friday, 8:00 am to 5:00 pm, with occasional backup for weekends and on-call responsibilities.
• Required Qualifications: Valid nursing license, Bachelor`s or Associate`s degree in Nursing, Colorado state licensure, and 1 year of nursing experience.
• Preferred Qualifications: Experience in hospice, palliative or end-of-life care, and certification in hospice or pain management is desirable.
• Additional Information: The primary registered nurse will manage an assigned caseload of patients, leading the interdisciplinary team to deliver personalized hospice care. Clinical excellence in assessment, symptom management, and critical thinking is essential. The role involves providing care wherever patients call home, including private residences, nursing homes, group homes, or assisted living facilities.
