Job Summary
The case management specialist is based in Indianapolis, Indiana. This role involves coordinating and overseeing patient care plans and ensuring effective communication among healthcare providers. Job Details

• Schedule: Day shift, 12 hours per shift, with a minimum of 36 hours per week
• Required Qualifications: 2 years of recent experience in case management or specialty management, appropriate licensure for Indiana or multi-state compact, ACLS and BLS certifications (must be through the American Heart Association) attached to the submission, vent/trach experience, and all certifications on file
• Preferred Qualifications: Not specified
• Additional Information: Candidates must have recent hospital-based experience in case management, and all submissions require the specified documentation.
