Job Summary:
Registered Nurse (RN) specializing in Case Management is sought for a position located in the Bronx, New York. The role involves providing care coordination, utilization review, and discharge planning services within an acute care setting. Job Details:

• Schedule: Days shift, 7.5 hours per day, minimum of 37.5 hours per week
• Required Qualifications: - Five (5) years minimum of clinical experience as a Registered Nurse - Experience with Care Coordination, Utilization review, and discharge planning - Computer literacy - Associates Degree (ASN, ADN) from an accredited Nursing Program - RN Licensed as a Registered Nurse in the State of New York - PRI and Screen certification within three months of employment - Primary Source Verification of License and Registration of Registered Professional Nurse from NYS Department of Education (NYSED) - BLS certification
• Preferred Qualifications: - Bachelors or Masters in Nursing - Case Management Certification - Leadership experience
• Additional Information:
  Candidates must obtain PRI and Screen certifications post-offer. Verification of license and registration must be completed through the NYS Department of Education. The role is located in the Bronx, New York, and involves providing high-quality case management services in a clinical environment.
