Registered Nurse / Case Management Exp

Job Summary:
Registered Nurse (RN) specializing in Case Management in New York, NY. This in-person position entails full responsibility for discharge planning from inpatient units, with a focus on patient and family communication regarding post-acute arrangements. Knowledge of hospice and end-of-life care principles is considered a bonus. Candidates should have hospital discharge planning experience and ideally five or more years of acute care case management experience.

Job Details:

• Schedule: Days shift, 8 hours per day, Monday through Friday, with a minimum of 37.5 hours per week.
• Required Qualifications: Certification not required; candidates must have experience in hospital discharge planning. Minimum experience of 0 years; preferred 5 years of acute care case management experience; location in New York, NY.
• Preferred Qualifications: None specified.
• Float: Float rotation may be required no.
• Additional Information: This position is fully in-person and does not include utilization review functions. Candidates must possess hospital discharge planning experience.
