Job Summary
Case Management Specialist in the Bronx, New York. This position involves discharge planning and making referrals within an acute care setting. The role requires collaboration with interdisciplinary teams to facilitate patient discharges and coordinate post-hospital care, primarily on Medical-Surgical or Telemetry units. The candidate should possess a positive attitude, strong critical thinking, and excellent time management skills.

Job Details

• Schedule: Days shift, 7.5 hours per shift, with a minimum of 37.5 hours per week.
• Required Qualifications: BLS Certification, Patient Review Instrument (PRI) Certification, SCREEN Training Certification; 1 year of inpatient case management experience; NY State license to submit.
• Preferred Qualifications: Knowledge of CarePort, MCG, and/or InterQual; prior utilization review experience.
• Additional Information: The role focuses on discharge planning for inpatient patients, including discharges to home, community, or skilled nursing facilities. Responsibilities include obtaining housing resources, shelter packets for homelessness, hemodialysis care plans, KCI referrals for wounds vacuums, and more. A resource binder is provided. The position involves working on acute care units with EMR documentation using Allscripts. The initial orientation includes two days of hospital orientation plus one to two weeks with a preceptor, depending on the candidate`s progress. Occasional weekend and holiday coverage may be required with manager approval. Scrubs should be business casual or any color except royal blue or maroon; a lab coat is provided.
