This case management position is located in Annapolis, Maryland. The role involves overseeing care coordination for the inpatient population within a healthcare organization. The Patient Care Coordinator collaborates with physicians, nursing staff, utilization review, and other healthcare team members to ensure seamless care progression, clinical efficiency, and cost-effective patient outcomes.
Job Details:
