Registered Nurse / Case Management Exp

Job Summary:
The organization is seeking a Case Management Professional for an assignment in Glen Burnie, Maryland. The role involves overseeing discharge planning and care coordination for acute inpatient populations, collaborating with healthcare teams and community providers to ensure safe and efficient patient transitions. The position requires hands-on expertise in discharge planning, case coordination, and proficiency with multiple electronic medical record systems.

Job Details:

• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week.
• Required Qualifications: Must have at least 1 year of experience in an acute medical, surgical, emergency department, or critical care nursing area. Valid license and certifications including Basic Life Support (BLS).
• Preferred Qualifications: Experience with discharge planning and care coordination. Ability to work across multiple electronic medical record systems.
• Additional Information: The role involves performing face-to-face assessments within 24 hours of admission, coordinating post-acute care referrals (such as home health, durable medical equipment, skilled nursing facilities, sub-acute, and acute rehab), and documenting care plans in accordance with organizational and regulatory policies. The candidate must be adaptable to facility-specific procedures and regulations to facilitate seamless patient transitions.
