Job Summary:
The organization is seeking a Case Management Registered Nurse (RN) with discharge planning experience in an acute care setting. The position involves coordinating care transitions, working collaboratively with physicians, social workers, and interdisciplinary teams to ensure timely patient progress and prevent unnecessary admissions or readmissions. This role is based in Oakland, California, and requires a focus on care coordination and discharge planning for adult and geriatric patient populations.
Job Details:
- Schedule: Day shift, 8 hours per shift, with a minimum of 40 hours per week
- Required Qualifications: CA RN license, BLS certification, at least 5 years of RN case management experience with inpatient acute hospital experience, discharge planning expertise, and experience with discharge processes (specifically discharge planning)
- Preferred Qualifications: Previous charge nurse experience, experience in community hospital, LTAC, or trauma level I or II settings, travel experience not required
- Float: Float rotation may be required
- Additional Information:
- Responsible for care coordination and care transition planning throughout the acute patient care experience, collaborating with multiple healthcare disciplines to optimize patient care and resource utilization.
- Conducts initial and ongoing patient assessments, reviews physician care plans, and manages the discharge process including post-acute care referrals.
- Monitors length of stay, re-evaluation of observation patients, and ensures compliance with regulatory requirements for transitions of care.
- Addresses clinical and social barriers to care, including psychosocial issues, and makes appropriate referrals.
- Actively participates in team meetings and ongoing department operations, contributing to practice improvements.
- Candidate must demonstrate effective communication skills, cultural sensitivity, and professionalism in a fast-paced environment.
