Job Summary
The healthcare organization in Mission Hills, California is seeking a Hospital Case Manager in the specialty of Case Management Experience. This position involves performing care coordination and discharge planning within a hospital setting. Candidates must hold a current California state license, or have a pending license that is active upon start date. Applicants are required to bring their license on the first day of employment.

Job Details

• Location: Mission Hills, California
• Shift Time: Days
• Shift Hours: 8 hours per day, with a minimum of 40 hours per week
• Start Date: June 22, 2026
• Required Qualifications: Certifications in acute care hospital case management, including a California state license (pending license accepted, but must be licensed upon start). A minimum of 2 years of current hospital case management experience performing care coordination and discharge planning, specifically in an acute care hospital.
• Preferred Qualifications: Experience with CMS (Centers for Medicare and Medicaid Services), utilizing InterQual Criteria, working with DRG (Diagnosis Related Groups), and navigating electronic medical records such as Epic.
• Additional Information: Must commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year`s Eve, New Year`s Day). References are requiredtwo references covering one year (six months for CNAs) within the last three years, including potentially Charge RNs or Lead RNs.
• Note: All travelers are expected to float between all facilities within the state. Refusal to float may result in termination or forfeiture of guaranteed hours.
