Job Summary
Case Management Registered Nurse (RN) opportunity in Mission Viejo, California. This position requires an experienced healthcare professional to provide care coordination, discharge planning, and patient assessment for adult and geriatric populations in an acute care setting. The role involves working days, with a schedule of 8-hour shifts and a minimum of 40 hours per week.
Job Details
- Schedule: Days, 8-hour shifts, minimum 40 hours per week
- Required Qualifications: 2 years of acute medical care management experience performing care coordination; California state nursing license (pending accepted, must be licensed upon start date); 2-year experience in case management; experience with assessment of patients, care coordination, discharge planning, and knowledge of healthcare systems such as HMOs and IPAs.
- Preferred Qualifications: Experience with trauma levels I and II, Epic charting system, and familiarity with patient populations experiencing homelessness, mental health crises, or substance abuse treatment options.
- Additional Information: The assignment involves working with adult and geriatric patient populations, with daily census up to 25 patients. The role requires independent coordination of DME, SNF, and ambulance transport arrangements, as well as participation in multidisciplinary team meetings and treatment plan development. Must be competent in compliance with HIPAA and other healthcare regulations.
- Travel Experience Required: Yes, travelers are expected to float between facilities within the state. Refusal to float may result in termination of assignment and forfeiture of guaranteed hours.
- Other Requirements: Must submit upcoming interview availability, confirm DOB and SSN in Connect, complete skills checklist, and provide two references covering 1 year within the last 3 years. RTO (Return to Office) will be limited to 7 days, and coverage includes working 3 of the 5 major holidays.
