Home Health - Nursing

Description

We are seeking a compassionate and self-driven Home Health Nurse to join our patient-focused community care team. In this rewarding and highly autonomous role, you will deliver skilled nursing services directly in patients’ homes, supporting individuals recovering from illness, surgery, chronic conditions, and mobility limitations while helping them maintain independence and comfort.

Your primary responsibilities will include conducting comprehensive in-home assessments, administering medications and IV therapies, managing wounds, monitoring vital signs, and coordinating care plans tailored to each patient’s unique environment and needs. You will also educate patients and families on disease management, medication safety, equipment use, and lifestyle adjustments that promote long-term wellness. Clear documentation, timely communication, and close collaboration with physicians, therapists, social workers, and caregivers are essential to ensuring continuity of care.

This role requires strong clinical judgment, excellent time-management skills, and the ability to work independently while providing warm, empathetic support. You will often be the primary clinical contact for patients, making reliability, problem-solving, and patient-centered communication critical.

Requirements and Qualifications

- Active RN license

- BLS certification (American Heart)

- Minimum of one (1) year of experience required for Olaro; individual facilities may require additional experience
